What To Expect

What to Bring to Your Appointment

  • Your insurance card
  • Physician referral forms, if required by insurance
  • List of prescriptions and/or over-the-counter medication, including dose and frequency
  • Information about your medical and surgical history
  • Recent x-rays or relevant records

Insurance and Payment Information

Statements are sent out monthly. If you have questions or are not able to pay your bill within ten days of receipt, please call our office at 605.217.7010 to make arrangements. We do file insurance claims for you and want to help you receive reimbursements from your insurance company. If you encounter problems with your insurance company, please contact us. We can help.

Siouxland Urology Associates participates with Medicare. Patients should confirm our participation status with their insurance company or managed care plans. Again, if you would like help, please contact us.
